Formal EAP Referral

When work performance or behaviour violates company policy, the supervisor/manager requires the employee to contact Mohawk EAP/Lifeworks through a Formal Referral process.

Mohawk EAP/Lifeworks will help the employee identify and resolve issues that create the performance and/or behaviour problem. With a Formal Referral, and the employee's signed informed consent, the supervisor/manager will be informed that the employee did or did not contact Mohawk EAP/LIfeworks, and is or is not in compliance with a plan established to resolve the work performance or behaviour problem. The content of the session is not disclosed at any time.
